CLE's Ordering Instructions for the Family

(THE FOLLOWING TWO PAGES ARE REPLACEMENTS FOR PAGE 14 OF THE "CLE ORDER FORMS" BOOKLET FOR FAMILY MEMBERS ONLY.)

(How to use the "Family's CLE Order Summary")

Follow the steps below to order by mail:

1. Account Information. Fill in the blanks. "Person Ordering" can be the name of the Home or of the individual family to receive materials being purchased with this order form.

2. Catalog Items. If you are ordering items from CLE's catalog that are not listed in the CLE Order Forms booklet, or if you are ordering only a few items, you may list them here and send in the "Family's CLE Order Summary " form only. It is essential that you list the "item #" and not just the name of the item.

3. Order Total. Discounts for the Family need to be calculated before filling in section #3 "Order Total" on the "Family's CLE Order Summary." (The Family discount for Lightunits and Answer Keys is 15%. A 10% discount is given for supplementary items and all other materials or CLE publications.)

In the CLE Order Forms booklet:

>> First please calculate these discounts on each individual order form page that you are ordering from.

>> There is no designated space on each order form page to figure these two discounts. So please just figure it out by writing the following after each section you are ordering from:

After the section "Individual Lightunits and Answer Keys":

— Discount 15% =_______

After the section "Lightunit and Answer Key Sets":

After the section "Supplementary Items":

>> If you have purchased materials from both the Lightunits and Answer Keys section (15% discount), and the Supplementary Items section (10% discount), it will be necessary to make a subtotal of these, as noted above. After you calculate your discounts, then add these discounted subtotals together to give the "Grade Total."

>> If you have ordered from both grades listed on the page you must then add together these totals. Then write in this combined "Page Total" on the line provided at the bottom of each order form page.

>> Transfer the "Page Total" from the bottom of each page of the order booklet to the "Family's CLE Order Summary," section #3 "Order Total." Then add these figures to get the subtotal of your complete order. Next, add the cost of shipping and handling (see point 4) for your grand total. If the order is to be shipped to a Virginia address, add 4.5% sales tax. If it is to be shipped to Canada, add 7% GST.

4. Shipping and Handling. Minimum charge is $2.50. Use Charts 4a and 4b to determine your cost.

For "Lightunits and Answer Keys," refer to Chart 4a and enter this total on the line given for Chart 4a. (After "# of LU/AK" fill in the total number of Lightunits and Answer Keys ordered. After the "+" put the amount indicated on Chart 4a.)

For "Supplementary Items" or any other materials, refer to Chart 4b. Go back through the booklet pages and add together all the "Subtotal: Discounted Sup. Items" (as mentioned above in point 3). To this figure add the total discounted catalog items (as per point 2 above "Catalog Items.") From this total calculate 10% which will give you the shipping costs for these materials. Enter this 10% on the line given for Chart 4b. (Please refer to "Sales Terms and Policies" on the next page for more details, including combining orders to save funds on shipping.)

5. Payment Method. Please obtain a money order, cashiers check or bank draft (drawn on a U.S. bank) made payable to Christian Light Publications in U.S. DOLLARS ONLY. Sorry, CLE cannot accept personal checks. Please see "Sales Terms and Policies" on the next page. Double check that payment covers both the materials and shipping.

6. Mail your completed booklet to the address on the right. (We suggest you keep a photocopy of your order.) You will receive a new booklet with your order. If you have marked only a few pages, you may tear out those pages and send them with the "Family's CLE Order Summary" form.

SALES TERMS AND POLICIES

GENERAL INSTRUCTIONS

— Family members' orders and payments should be mailed to your Family distributor's address and not directly to CLE.

— Please use CLE Order Forms booklet, filling out those pages which contain the materials you wish to purchase; however do not use page 14 and 15 with the Ordering Instructions and CLE Order Summary. Family members should only use the customized "Family Ordering Instructions for CLE" and "Family's CLE Order Summary" form that has been made for Family members' orders.

— Please photocopy the "Family's CLE Order Summary" form in order to have blank forms on hand for future ordering.

— Once the Family distributor receives your order, they will double-check the orders for accuracy and for correct payment, after which the order will be sent along with your payment to CLE.

— Establish a shipping name and address when placing your first order, and as much as possible use the same name and address for further orders. If in the U.S., the shipping address should include a complete address suitable for both Parcel Post and UPS. If there is a change of address, please include the old address.

— The Family account code is made up mostly with the "Family" name, "FAMILYDCO" and will be found on all shipping labels, invoices, and statements. Please include this code, "FAMILYDCO" on all correspondence.

— All CLE order forms may be photocopied.

— Family members may select any of the three CLE service options as described in CLE's "Home Study Options" brochure i.e., Full Program, Training Without Services, or Curriculum and Materials.

PAYMENT

— Total payment must cover the cost of the material as well as the shipping and handling.

— Payment must be in U.S. DOLLARS in the form of a money order, cashier's checks or bank draft (drawn on a U.S. bank).

Orders received without payment cannot be forwarded to CLE.

— If payment received has mistakenly been less than what is ordered, so as to not delay your order, an adjustment will be made by subtracting materials to equal payment received. If payment received is more than the total cost of the materials and shipping costs, then CLE will return the excess funds by letter directly to the shipping address given on your order form.

— Canadian customers receiving packages by UPS may need to pay extra charges when the delivery is made.

SHIPPING

— A special shipping rate has been designed for members of The Family. This rate is shown on the "Family's CLE Order Summary" in Chart 4a and 4b. These charts may be adjusted from time-to-time in an effort to keep CLE's average cost of shipping and packaging of Family orders equal to actual costs.

— To save freight costs, Family Homes, when and where it is feasible, are encouraged to combine orders into a single shipment to one address. In other words, several Homes in one area may choose to compile their individual orders on to one set of order forms including using only one "Family's CLE Order Summary" sheet. The materials purchased on the order form pages and the totals listed on the order form summary sheet would be the totals of the orders from several different families or Homes compiled into one set of order forms. Since all the materials in these orders will then be packaged together, to distinguish between the individual orders, be sure to keep a good record of the materials ordered by each Home or family. (It is suggested to keep a photocopy of orders made.)

Please plan your orders to allow at least two months for delivery for those overseas.

— Rush orders are not encouraged, but if you choose this, the shipping cost is 20% of the cost of your order for U.S. Homes and 25% for foreign delivery. Plus, when you choose a rush order shipment, your discount on the Lightunits and Answer Keys will be reduced from a 15% discount to only a 5% discount. Rush orders are usually sent UPS to U.S. addresses taking 10-15 days for delivery. Foreign rush orders are handled differently. Rush orders to a foreign address are immediately processed through CLE's office and shipping department, taking priority over all other orders, but all foreign orders are shipped surface mail, not by air mail parcel post or UPS.

CHECKING ORDERS/ADJUSTMENTS

— An itemized packing slip will be enclosed with each shipment, and the outside label on each box will show the number of boxes in the shipment. (Example: for a 3-box shipment: 1 of 3; 2 of 3; 3 of 3.) Upon arrival of your shipment, please check that you have received the correct number of boxes, and compare the entire contents received along with the enclosed packing list.

— Items temporarily out of stock will be circled on the packing list and sent as soon as they become available.

— In the event that there is an error in packaging, please immediately fill in the "Quick Correction Form" that will be enclosed with each shipment. Send this completed form directly to CLE.

LOST OR DAMAGED SHIPMENTS

CLE warrants that all orders will be in good condition when shipped, and that they will be packaged according to normal U.S. Postal service and United Parcel shipping standards. In the event that merchandise arrives in a damaged condition, it is the recipient's responsibility to file a claim with the postal service or shipping company for all lost or damaged orders. CLE will not send a second shipment free if shipment is lost or damaged. CLE's responsibility will be limited to providing proof of shipment and substantiating the value of the shipment. Claims must be filed within 30 days of receiving your package.

RETURNS

— Generally, returns are not accepted. Special consideration for an exception will be considered upon written request. Except for CLE errors, no request will be considered for returns having a value of less than $100.00, or for items shipped more than four months prior to receiving the request.

— If you wish to make such a request, do so by writing a letter directly to CLE. If approval is given, CLE will send directly to you a letter of approval of a full payment credit and give you a Return Merchandise Authorization number. CLE will not give credit or a reimbursement for returned merchandise for any items returned without having this authorization number enclosed with returned merchandise.

— All returns must be received in saleable condition, e.g., cassette tapes without seal broken, Lightunits or Answer Keys without tests removed or written in, etc.

— Since there is a four-month time limit for returning materials, CLE will accredit you from the date they receive your letter of request to return your materials, and not the date they would actually receive the returned merchandise.
